Claiming an EVGA rebate?

I just got my GTX 460 from EVGA and am now applying for the rebate. I filled out all the information and printed out the rebate form. I've got the packing list and the rebate form and it says I need the 'Original UPC/Serial Number Barcode Label'. Is that the sticker on the back of the box? Do I just cut that off and put it in the envelope? It also says to cut and use the mailing label that was printed on the Rebate Form, but I'd rather just write down the mailing address. Is it alright if I do that instead?
